Participant Consent Form

Everyone participating in a lesson or event with Barefoot Vibes Paddleboard School must complete a Participant Consent Form. The form can be completed online here or downloaded using the button below to complete offline and returned by email or in person.

Declaration

  • I have declared all medical, physical or mental conditions which could endanger myself or others.

  • I understand that Barefoot Vibes staff will administer emergency medical treatment without consent in circumstances where those requiring urgent medical assistance are unable to provide consent and no parent or guardian is present.

  • I acknowledge that any participant under the age of 18 must be accompanied by a responsible adult on the water for the whole session.

  • I agree to follow the instructions and directions of the School's staff and instructors at all times during the lesson or tour.

  • I am aware that all participants wear the appropriate clothing and equipment provided or recommended by the School, such as a wetsuit, buoyancy aid, helmet, and paddle board.

  • I will respect the rights and safety of other participants, staff, instructors, and third parties during the lesson or tour.

  • I will not consume any alcohol, drugs, or other substances that may impair the Customer's ability or judgment before or during the lesson or tour.

  • All participants I am responsible for are medically fit to participate, be able to swim confidently, be relatively fit, and be to move between kneeling and standing without difficulty.

  • I am aware of the risk of waterborne diseases, including Weil's Disease (leptospirosis), and take sensible precautions including: covering any cuts or scratches with a waterproof covering; If you fall in, take a shower and treat cuts with antiseptic and a sterile dressing; wash wet clothing before you wear it again; and if you develop flu-like symptoms within two weeks, see a doctor and mention that you fell in the water and the risk of Weil's Disease.

  • I have informed the School of any changes in the medical condition or special requirements of all participants I am responsible for.
